تاثیر 8 هفته تمرین HIIT و مکمل یاری کورکومین بر میزان آدیپونکتین و مقاومت انسولینی در زنان چاق مبتلا به دیابت نوع 2

چکیده
مقدمه و هدف: آدیپوکین­ ها اعمال حیاتی زیادی را تنظیم می­ کنند و نقش مهمی در پاتوفیزیولوژی مقاومت به انسولین، دیابت، اختلال در اندوتلیال عروقی و التهاب ایفا می­ کنند. فعالیت ­بدنی و مصرف گیاهان دارویی با خواص ضدالتهابی نیز یکی از پرکاربردترین روش ­ها برای کاهش عوارض دیابت نوع دو است. از این رو هدف از تحقیق حاضر بررسی تاثیر 8 هفته تمرین HIIT و مکمل یاری کورکومین بر میزان آدیپونکتین و مقاومت انسولینی در زنان چاق مبتلا به دیابت نوع 2 بود.
مواد و روش ­ها: پژوهش حاضر از نوع مطالعه­ شبه ­جربی بود که به صورت تصادفی 40 نفر از بین 60 نفر از زنان مبتلا به دیابت نوع 2 با میانگین سنی 0.13±36.5 و میانگین وزنی 3.53±89.6 کیلوگرم، ساکن شهرستان ارومیه در سال 1403-1402 انتخاب شدند. مشارکت کنندگان در مطالعه به صورت تصادفی ساده و در دو گروه همگن 20 نفری شامل: گروه تمرین مکمل و گروه تمرین دارونما تقسیم شدند. مداخله تمرینی سه جلسه در هفته و مصرف کورکومین روزانه 2100 میلی­‌گرم در سه وعده 700 میلی­‌گرمی به مدت 8 هفته انجام شد. قبل و بعد از تمرین شاخص‌­های بیوشیمیایی و ترکیب­‌بدنی اندازه‌­گیری شد. نمونه‌­های خونی در طی دو مرحله (پیش­ و پس­‌آزمون) برای اندازه­‌گیری متغیرهای بیوشیمایی جمع‌­آوری شدند. داده­‌ها با استفاده از آزمون تحلیل کوواریانس و در سطح معنی‌­داری 0.05 بررسی گردید.
یافته­ ها: در گروه تمرین+ مکمل کورکومین، اثر تعاملی کورکومین و تمرین تناوبی شدید موجب افزایش آدیپونکتین (P=0.001)، کاهش مقاومت انسولینی (P=0.001)، کاهش انسولین (P=0.001)،  و کاهش معنی­دار گلوکز (P=0.002) شد، در حالی که در گروه تمرین+دارونما این تغییرات معنی دار نبود.
 بحث و نتیجه­ گیری: تحقیق حاضر نشان می­دهد که انجام تمرینات تناوبی شدید به همراه مصرف کورکومین به عنوان یک روش غیرتهاجمی می­تواند اثر مثبت و مهمی بر افزایش آدیپونکتین و کاهش مقاومت انسولینی در بیماران مبتلا به دیابت نوع 2 داشته باشد.

Introduction and Purpose: Adipokines play an important role in complications related to type 2 diabetes and regulate many vital actions that play an important role in the pathophysiology of insulin resistance, diabetes, sweat endothelial disorders and inflammation. A drug with anti-inflammatory properties is also one of the most widely used methods to reduce the complications of type 2 diabetes, the aim of this research was to investigate the effect of 8 weeks of HIIT training and curcumin supplementation on adiponectin levels and insulin resistance in obese women with type 2 diabetes.
Materials and   Methods: The present study was a semi-experimental study. 40 people were randomly selected among 60 women with type 2 diabetes with an average age of 36.5±0.13 and an average weight of 89.6±3.53 kg, living in Urmia city. Participants in the study were randomly divided into two homogenous groups of 20 including: the placebo supplement training group and the placebo training group. Exercise intervention three sessions a week and daily curcumin consumption of 2100 mg in three 700 mg meals was performed for 8 weeks. Biochemical and body composition indicators were measured before and after exercise. Blood samples were collected during two stages (pre-test and post-test) to measure biochemical variables. The data were analyzed using the analysis of covariance test of SPSS20 software and at a significance level of 0.05.
Results: In the exercise group + curcumin supplement, the interactive effect of curcumin and intense intermittent exercise increased adiponectin (P=0.001), decreased insulin resistance (P=0.001), decreased insulin (P=0.001), and significantly decreased glucose (P=0.002). (P=0) while in the exercise+placebo group, these changes were not significant.
Discussion and Conclusion: The current research shows that performing intense intermittent exercises along with curcumin as a non-invasive method can have a positive and important effect on increasing adiponectin and reducing insulin resistance in patients with type 2 diabetes.
