تاثیر تمرینات پیلاتس در خانه و زیر نور آفتاب بر عملکرد قلبی‌ تنفسی و برخی عوامل آمادگی جسمانی در زنان مبتلا به مولتیپل اسکلروزیس

مقدمه و هدف: مولتیپل اسکلروزیس (ام اس) یک بیماری ناتوان‌کننده سیستم عصبی است که سبب ضعف عضلانی و اختلال در عملکرد جسمانی از جمله راه‌رفتن و تعادل می‌شود. هدف مطالعه حاضر مقایسه تمرین پیلاتس درخانه و زیر نور­آفتاب بر میزان استقامت قلبی‌ تنفسی، قدرت و استقامت عضلانی، تعادل، هماهنگی و ویتامین ‌دی بیماران مولتیپل اسکلروزیس بود.
مواد و روش‌ها: از میان زنان مبتلا به ام اس تعداد 44 نفر به صورت تصادفی انتخاب و به سه گروه تمرین در خانه، تمرین زیر نور آفتاب و کنترل تقسیم شدند. گروه‌های تمرینی 8 هفته تمرین پیلاتس را بین ساعت 8 تا 10 صبح به صورت انفرادی (از طریق DVD) در خانه یا زیر نورآفتاب انجام دادند. استقامت قلبی‌تنفسی و استقامت عضلانی، قدرت بالاتنه و پایین‌تنه، تعادل، هماهنگی و سطح ویتامین‌ دی بیماران پیش و پس از تمرینات اندازه‌گیری شد و داده‌ها با استفاده از روش آماری کوواریانس تحلیل شدند.
یافته‌ها: هشت هفته تمرین پیلاتس در خانه و زیر نور آفتاب باعث افزایش استقامت قلبی‌تنفسی و استقامت عضلانی، قدرت بالاتنه و پایین‌تنه، هماهنگی و سطح ویتامین‌ دی و بهبود تعادل بیماران شد (0.05>P < span lang="FA">). نتایج آزمون تعقیبی نشان داد که تمرین زیر نورآفتاب در مقایسه با تمرین در خانه باعث افزایش بیشتر در قدرت عضلانی بالاتنه و پایین‌تنه، تعادل و ویتامین‌ دی بیماران شد. اما تغییرات در میزان استقامت قلبی‌تنفسی، استقامت عضلانی و هماهنگی بین دو گروه تمرینی از نظر آماری معنی‌دار نبود (0.05).
بحث و نتیجه‌گیری: به طور کلی تمرین پیلاتس در خانه و زیر نورآفتاب باعث افزایش ظرفیت عملکردی زنان مبتلا به ام­اس شده و جهت اثرگذاری بیشتر، تمرین زیر نورآفتاب به‌علت کاهش عوارض جسمانی بیماری توصیه می‌شود.

Introduction and purpose: Multiple Sclerosis (MS) is a debilitating disease of the nervous system that causes muscle weakness and impaired physical function, including walking and balance. The aim of this study was to investigate cardiovascular endurance, muscle endurance, upper and lower strength, coordination, balance and vitamin D levels in MS patients after Home-based and Outdoor Pilates training.
Materials and Methods: 44 women with MS were randomly selected and divided into 3 groups: home-based training (HPT), outdoor training (OPT), and a control. The training groups performed 8 weeks of Pilates between 8 and 10 a.m. individually (via DVD) at home or under sunlight, cardiovascular and muscular endurance, upper and lower body strength, balance, coordination, and vitamin D level of the patients were measured before and
after the 8-week training. The data were analyzed using the covariance statistical method 
Results: Eight weeks of HPT and OPT increased cardiovascular and muscular endurance, upper and lower body strength, coordination, and vitamin D level, and improved the balance of patients (P<0.05). The post-hoc analysis revealed that, compared to HPT, OPT caused a greater increase in upper and lower body muscle strength, balance, and vitamin D in patients. However, the changes in cardiovascular and muscular endurance and coordination between OPT and HPT were not statistically significant (P>0.05).
Discussion and Conclusion: In general, HPT and OPT increase the functional capacity of women with MS, and to be more effective, OPT is recommended due to the reduction of physical complications of the disease.
